Assisted Living Services in Corona, Queens

Daily support at Madison York

Madison York Assisted Living Community is at 112-14 Corona Avenue in Corona, Queens, NY 11368. Our adult-home services and Assisted Living Program (ALP) support residents with different daily needs. Talk with our team about the services and assessments relevant to your situation.

Room and board includes three meals and two snacks each day, case management, assistance with self-administration of medication, housekeeping, laundry, maintenance, and activities. Resident rooms do not have kitchenettes.

Personal care and nursing support

For eligible ALP residents, services include 24-hour supervision and help with daily activities such as toileting, hygiene, eating, and transfers, according to assessed needs. York Home Care registered nurses supervise activities-of-daily-living and home-health-aide services daily. They also assist with or supervise prescribed fingerstick testing, insulin, and other injectable medications.

Assessments and medical visits

Assessments take place as needed, at least annually for adult-home residents and at least twice a year for ALP residents. On-site physicians and York Home Care nurses are available for acute or intermittent assessments between these reviews. Residents who choose a physician with privileges at the facility can access on-site visits twice a week. These physicians review orders, referrals, and overall care.

Transportation to appointments

Ambulette transportation to medical appointments is available for Medicaid and private-pay residents, with ambulance transportation for acute hospitalization. Case managers can assist with applications for MTA Access-A-Ride. Ask our team about arrangements, eligibility, and any charges for the transportation you need.

Madison York accepts straight Medicaid for eligible ALP residents’ supervision and assistance with daily activities. Outside private payment, Medicaid is the insurance source currently accepted for ALP services. This does not describe coverage for separate physician visits or other medical care.

Read our Medicaid and paying-for-care guide, then contact our admissions team to review current charges, eligibility, and the services covered by your payment arrangement. Admission and coverage depend on individual assessment and program requirements.
